Moreover, providers have always cited significant barriers to patients’ consistent engagement and participation in health care. They indicate that patients forget or miss clinic appointments associated with clinical consultants, follow-up laboratory tests, and regular clinic visits for various reasons, including changes in clinical staff, distance, and time (Clark, 2015). Thus, the findings of the assessment mirror remarkable issues in accessing quality services for adults with diabetes. 4 Healthy People 2020 Goals Related to the Topic A broad combination of Healthy People 2020 goals relates to the topic and primarily revolves around preventing and managing diabetes. For instance, Healthy People 2020 aims to reduce the burden caused by the disease, particularly for patients and their families (Healthy People 2020). Most families lack sufficient resources to address the negative impacts of the disease and fall into debt and poverty. Improving the quality of life for people with diabetes and those at risk is the other Healthy People 2020 goal associated with the topic (Healthy People 2020). Another related Healthy People goal is reducing the annual number of people diagnosed with new cases. This goal is achieved by creating physical and social environments promoting good health. Additionally, reducing the death rates associated with the disease is central to Healthy People 2020 goals linked to the topic. The described goals are being pursued by promoting continuity of care with a fundamental focus on access to specialized care and emphasis on patient-provider relationships; addressing mental health needs, engaging communities and families in diabetes management and prevention; addressing challenges associated with healthy eating, and addressing misconceptions and myths of adults with the disease (Clark, 2015). Therefore, Healthy People 2020 provides instrumental goals associated with the described topic based on their focus on improving the quality of life, reducing deaths, and reducing the burden caused by the disease. Establishing diabetes control and prevention programs is perhaps the most overlooked Healthy People 2020 goal associated with the topic. For instance, the Texas Department of State Health Services Diabetes Prevention mainly oversees state and federal programs emphasizing diabetes education for professionals and the public (Healthy People 2020). The department aims 5 at assisting individuals with diabetes in managing the disease and the outcomes if it is not treated; delaying or preventing complications in people with the disease; delaying or preventing the onset of the illness in individuals with various risk factors, including gestational diabetes and prediabetes, and preventing the disease in individuals at high risk (Healthy People 2020). It has established numerous mechanisms to achieve the described outcomes, including disseminating educational materials to health care providers and the general public, providing financial support, and strengthening existing mechanisms. Similarities and Differences in Data Data that was gathered and the data that was generated have notable similarities and differences in terms of prevalence and distribution. The data was drawn from the Texas Demographics Center and replicated within Katy city (Texas Demographic Center, 2020). Both data sources revealed that the Cinco Ranch community is below the US prevalence for diabetes. The data further showed that the prevalence of the described disease is unevenly distributed across the state. Specifically, the highest prevalence rates were found among non-Hispanic Blacks, Hispanics, and Alaskan or Indian natives. Almost 12.4% of the adult population, or approximately a 2.7million people, have been diagnosed with diabetes in Texas (Texas Demographic Center, 2020). More than 34% of the total adult population have prediabetes with high blood sugar levels that is not adequate to be diagnosed as diabetes. Strengths of the Community Considerable access to the healthcare system, accessibility to high education, and job and population growth are some of the strengths of the examined community. The community enjoys healthy population growth (Houston Newcomer Guides, 2022). However, managing this steady 6 growth presents a new set of challenges, most of which are in the initial stages, especially if the growth maintains the projected trend. Notwithstanding, a high-skilled and well-developed workforce, local colleges, dedicated community leaders, and careful planning has a promising future for the health and general well-being of the community. Moreover, the community in contention enjoys a low cost of living compared to others, which is one of the most profound and compelling community assets (Texas Demographic Center, 2020). Specifically, it takes minimal time to access higher education or healthcare compared to other communities. The community’s robust transportation systems also provide easy access to healthcare services. Moreover, the community is reaping huge benefits from the remarkable progress made in the state’s healthcare system. Precisely, the state has led the way in covering nursing home care via managed care plans for people with diabetes and other diseases who cannot afford Medicare (Texas Demographic Center, 2020). The community has also benefited from the state’s investment in new medical schools and the recruitment of foreign medical school graduates. Moreover, it has taken meaningful steps to address the challenge of surprise balance billing when individuals face substantial unexpected costs for poor room care services. Weaknesses of the Community Insufficient services, stringent Medicaid regulations, and a high uninsured rate are some key weaknesses associated with Texas state and find relevance in the Cinco Ranch community. Thanks to these weaknesses, Texas has one of the highest disease and death rates in the United States of America. Most people do not have health insurance in Katy city, which is clear in the evidence that almost 4.5 million people in Texas lack health insurance (Texas Demographic Center, 2020). The state has more uninsured citizens than California, even though the latter has more people than the former. The Affordable Care Act significantly reduced the number of 7 uninsured people by covering many residents. However, the coverage remains one of the worst in the entire United States of America. The state passed laws that severed the ability of communities to help individuals enroll and decided not to establish in-person assistance programs. Moreover, the region in question refused to accept federal dollars to assist those who do not qualify for subsidies, a huge weakness affecting healthcare delivery (Texas Demographic Center, 2020). The community is tied for the prize of being the region with one of the least income caps for individuals to qualify. In this regard, policymakers should establish mechanisms to address the challenges and weaknesses to ensure community well-being and health. Areas of Improvement Focus groups, the impact of culture, and identifying motivators of behavioral change are some key areas of improvement. Glucometers should be viewed as a robust incentive for people who have diabetes (Guo et al., 2020). Most adults with this disease value tracking their glucose patterns. These individuals should be encouraged to gain experience in testing and monitoring their glucose levels. Moreover, focus groups should be leveraged to determine the cultural satisfaction with and acceptability of interventions, obtain community input into intervention refinement and evaluation, and assess community needs to establish culturally-relevant interventions (Guo et al. 2020). Policymakers and other players in the healthcare systems should focus on the retention and recruitment of minority groups to allow the high representation of these groups in establishing crucial interventions (Clark, 2015). Simply put, a broad combination of improvement areas is available for the community and the entire state to consider. 8 Conclusion It is vital to consider mental health, insufficient resources, and lack of adequate family support aspects associated with diabetes to meet the Healthy People 2020 goals of improving the quality of life, reducing deaths, and reducing the burden caused by the disease. A keen focus on the key weaknesses associated with the Cinco Ranch community, including poor insurance coverage and limited awareness or education, as well as utilizing strengths associated with existing infrastructure and improved education, can allow the community to address some of its key health challenges. Healthcare institutions often select the desired group based on their characteristics and focus on serving them (Elrod & Fortenberry, 2018). A community health assessment was used to identify appropriate target groups. It assesses previously identified segments on the following criteria: magnitude of the problem, the problem’s severity, the community’s willingness and capacity to act on the issue, the availability of community resources, and whether the particular issue acts as a root cause of other problems. The target audience shares similar demographic factors such as age, income level, gender, marital status, education, occupation, and geographical location. These characteristics form a basis for arranging service attributes in ways that appeal to this particular group. Therefore, the information about the target audience will help define plans and strategies, including the messages and communication channels to execute, promoting better communication and relationships with clients. A community assessment in my chosen community will include data on demographics and health issues. As a result, demographic characteristics such as age, household income, gender, family status, and other demographic items will be used to identify the target audience. Also, the criteria will provide information on health issues such as diabetes, including the magnitude and severity of the problem. The community improvement processes will be prioritize based on the severity of this problem. Notably, priority will be granted to health problems with the highest severity. In this case, mental health, lack of family support, and insufficient resources, including medications, healthy food, and supplies, significantly affect the community 3 suffering with chronic diabetes. In addition, the prioritization process was guided by the community’s willingness and capacity to act on the identified issue. This criterion will evaluate how willing the community is to address the pressing health problem. Issues with greater community willingness will be prioritized. Connection to Target Audience Connecting with the target audience is vital for better understanding their needs and problems. Connection influences the audience’s willingness to address the health problem. My target audience is adults with diabetes that lack family support and insufficient resources. I have interacted with them at personal and professional levels. The group members are frequent visitors to my chosen site. Having interacted with them, I know most of them by their names. Also, some of them have introduced me to their family members and friends. Most importantly, I have a family member within the community who has diabetes, which allows me to have better understanding about audience experiences. Besides, I share professional attributes, including attention to detail, strong communication skills, coordination, problem-solving skills, empathy and compassion. Nearly all the community members demonstrated a high level of willingness and cooperation to act on the issues, citing it as the first step to positive community development.
